We are planning to visit the sakino-yu, just wondering, is the outdoor bath required a bathing suit? Are they a share bath? Do you think we can visit shirahama and kii-katsuura in the same day? we are staying at osaka.

No, bathing suits are not allowed.

No, they are gender separated.
Do you think we can visit shirahama and kii-katsuura in the same day? we are staying at osaka.
It is probably theoretically possible, but I would not recommend it to most types of travelers except the ones who want to see as much as possible within a short time frame and do not mind to spend much more time driving or on board of trains than sightseeing.
It also depends on how much you want to see in each place.

As a day trip from Osaka Shirahama is more suitable as the travelling time to and from Osaka is shorter. Kii-Katsuura in comparison has less to offer but it is the perfect base for the excursion to Nachi Falls which is considered a top destination in the Kii Peninsula.
