The core islands are Naoshima, Teshima, Inujima, and Shodoshima.
I've been to Naoshima and Inujima. I highly recommend both!

You can access from Uno or from Takamatsu (it's more convenient from Takamatsu). However, contrarily to my predecessing commentor, I'd advise to skip Naoshima and Inujima if you have the opportunity to go at another time, as there isn't really much difference during the triennale and when the triennale is not "on" except for more people and longer lines. It's on the other islands (Shodoshima, Teshima, Ogijima, Megijima, Oshima and the "new" ones: Shamijima in the Spring, Ibukijima in the Summer, Awajima and Takamijima in the Fall) that things are very special during the Triennale.
