Day 1 to 5 in Kyoto, flying into KIX
Day 6 in Hiroshima
Day 7 to 12 in Tokyo, flying out of HND

Activate JR Pass on day 6.

Would your teenage daughters prefer to see temples and shrines or shopping at trendy Japanese shops and fashion stores? Woud they prefer natural scenery or theme parks? Traditional accomodations and onsens or modern hotels?

I personally prefer the city sights vs the religious sights and would recommend basing yourself in Osaka for half the trip and Tokyo the other half. Thats assuming you're arriving in Osaka and departing Tokyo?

Once you've picked your hotels you can do day trips from there. This will limit the accommodation hopping with your luggage to a minimum.

From Osaka you can do day trips to Kyoto, Nara
and Hiroshima to name a few. Check out http://www.japan-guide.com/e/e2157.html

From Tokyo you can go to Nikko, Hakone and Enoshima. Check out http://www.japan-guide.com/e/e2164.html

But there are so much to see and do in Osaka and Tokyo alone that you don't need to go outside the cities (eg no need for JR Pass if on limited budget), but it would be a shame to miss out on some of the side trips when they are so easy to access.

Forget Himeji. The castle is under renovation and invisible under a tarp.
There is not much interesting sites to see in Kobe.
The JR pass is not profitable for your stay.
Take care, the weather in late June is often rainy in Japan (tsuyu). Do not book in advance to Hakone. Only go to see Fujusan if the weather is perfectly clear on Tokyo.

If you skip Hiroshima, any kind of JR Passes does not pay off.
