Yes, you have to pay a supplement for two reasons:
1) the limited express supplement (500 yen as of March 30, 2014) is not covered by the Kansai Thru Pass.
2) the Kansai Thru Pass is not valid all the way to Yoshino. The additional fare to be paid is 420 yen (as of March 30, 2014)
Due to the consumption tax increase, the supplements may be slightly higher from April 1.
