I have 2 Questions :

1) Should I purchase the Icoca card?
2) Should I visit Universal Studio and Aquarium in one day. Is there sufficient time?

I will be travelling with my family (4 of us) to Osaka, Kyoto and Nara. Our itinerary will be as follows :

Day 1 : KIX - Kyoto by Haruka Limited Express. Overnight in Kyoto.

Day 2 : Kyoto - Nara - Kyoto. Visit Todaiji temple, Nara Park, Nara Museum, Isuien Garden etc. Overnight in Kyoto.

Day 3 : Sight-seeing in Kyoto. Visit Nijo Castle, Nishiki market, Gion, etc. Overnight in Kyoto.

Day 4 : Kyoto - Osaka. Visit Dotonbori. Overnight in Osaka.

Day 5 : Osaka - Visit Universal Studio. Overnight in Osaka.

Day 6 : Osaka - Visit Aquarium, Osaka castle.

Day 7 : Osaka - KIX Airport.

Please advise.

You had better visit them on different days as you plan now, I suppose.
(You may need to wait in getting admission to Universal Studios Japan and for some attractions inside the park.)

One day sounds too short for touring Kyoto City, especially if it is your first time.
Usually you can travel from Kyoto City to Osaka City within one hour, so you can tour Kyoto from the morning, head to Osaka in the afternoon, check in and visit Dotonbori in the evening.

Unless you fly into KIX in very late or early hours, you can purchase a roundtrip ICOCA and Haruka package at the JR ticket counter (open 5:30 - 23:00) in Kansai-airport Station of JR-West.
The last Haruka train for Kyoto is scheduled to depart at 22:16.

Kyoto City Bus lines / Kyoto Bus lines are not ready for IC cards. You can purchase a (magnetic) one-day bus card for 500 yen, which pays off in 3 or more rides inside the valid zone.
The bus stops Nijo-jo mae, Shijo-Karasuma (near Nishiki Market) and Gion are inside the valid zone of this one-day card.

Kyoto Tourist Information Center (open 8:30 - 19:00) is along the south-north pedestrian walkway on the 2nd floor of Kyoto Station Building. West Gate is closest.

If your accommodation in Osaka City is far away, it is worth considering using a major delivery service. Dispatching your parcel in Kyoto City in the morning of Day 2, you can get it delivered to your Osaka accommodation usually by the night of Day 3, or maybe in the morning of Day 4.
