I'd like to have a travel plan for my family, 5 persons and my husband has to travel with wheelchair. So,we prefer the wheelchair accessible place. We start and end our trip from Chitose airport.

There are a lot of posts on Hokkaido on this forum... look around and see where you want to go, and how far/much you can drive in a day, then plan your route. You'll have to do research on what you want to, and can, do.
If you're wanting someone to do it for you, you'll have better results going with a travel company who can plan a custom tour for you. That way you also make sure there is wheelchair access everywhere you want to go.
