Here's a suggested itinerary for 6 days;
Day 1; Shinjuku and Shibuya (only Shinjuku would be enough for one day, depends on what you like).
Day 2: Odaiba & Tokyo station are (again, Odaiba has for a full day, it depends on what you like and your pace). I didn't find Ginza any interesting unless you like western-like posh areas.
Day 3: I'd choose Fuji 5 Lakes over Hakone, but it's up to you. No way to do both in a single day.
Day 4: Asakusa & Akihabara
Day 5: Kamakura & Yokohama or Nikko.
Day 6: Last minute shopping and leave?

What would you visit in Yoyogi? Hotel Sunroute Plaza Shinjuku is located in the northern part of Yoyogi district of Shibuya, around the boundary between Shibuya and Shinjuku. And, practically many facilities with "Yoyogi" in their names are located in a few districts along JR Yamanote Line, roughly to the west of its section Shinjuku -- Yoyogi -- Harajuku -- Shibuya.
A route which I think of for Day 1 is like the below. Hotel ---- Entrance A1: Shinjuku Station (S-01) --[Toei subway Shinjuku Line]-- (S-02) Shinjuku-sanchome Station of Toei subway --(walking)-- Shinjuku-sanchome Station (F-13) of Tokyo Metro --[Tokyo Metro (subway) Fukutoshin Line: Local train]-- (F-14) Kita-sando Station ---- Meiji Jingu Shrine: North Gate, Main Shrine Building, South Gate --Omotesando Street-- Omotesando crossing --(turning back)-- Jingumae crossing ---- Takeshita-guchi crossing --Takeshita-dori Street-- Harajuku Station --[JR Yamanote Linefs inner loop]-- Shibuya Station ---- Statue of the dog Hachiko ---- Shibuya Station crossing ---- Center-gai: Spain Slope, Tokyu Hands, Shibuya PARCO ---- Entrance 13: Shinjuku Station (F-16) --[Tokyo Metro (subway) Fukutoshin Line]-- (F-13) Shinjuku-sanchome Station: Exit E8 ---- Shinjuku Takashimaya ---- Shinjuku-sanchome Station (M-09) --[Tokyo Metro (subway) Marunouchi Line]-- (M-08) Shinjuku Station of Tokyo Metro --(walking)-- (E-01) Shinjuku-nishiguchi Station of Toei subway: Exit D1 ---- Shinjuku Omoide Yokocho ---- Shinjuku-nishiguchi Station (E-01) --[Toei subway Oedo Line]-- (E-28) Tochomae Station: Exit A4 ---- Tokyo Metropolitan Building No. 1: Observatories ---- Tochomae Station (E-28) --[Toei subway Oedo Line]-- (E-27) Shinjuku Station: Exit A1 -- Hotel.

As same as previus comment, First week of November is too early for autumn leaves in Tokyo. Tree leaves color still not be maple color generally. Maple viewing will start from mid of November. Not only Shinjyuku, but also any Tokyo spot.
