The US Embassy site recommends the following hospitals as English speaking, so you could contact them and ask for help.

There are many abortion clinics; Japan does not have the same angst with abortion that some countries have. Most OBGs should be able to point you in the right direction. AFAIK You will need to have a signature of a male to go ahead with the procedure. It is not covered by National Health.

With some exceptions, if you are married you'll need both partner's consent. Otherwise, you can go ahead with the procedure unilaterally.
