Pakistani passport?
Just call the Pakistan embassy in Tokyo, they will guide you to get one.

Japanese passport?
If both of you are foreigners there is no way to get a Japanese passport for just being born in Japan.

Don't forget to apply for the foreign passport within 30 days of birth. Also you must apply for residency status for your child within 30 days at your local immigration office.

Local registration must be done within 2 weeks at your city hall, they will take your hospital birth certificate at add the child to your juminhyo. If you are likely to need further copies of birth certificate for the future, then get them at the time of birth from the hospital. You will also have apply for a health card within 15 days of birth and register with your health insurance provider as well as later at city hall for medical card.

At some point you may need to apply for child allowance benefit and do the proof for your birth gift payment.
