I have written to directly a hotel in Yuzawa to make a room reservation.There is no respond. I need to write in Japanese to request them if my booking is confirmed.
How to write in Japanese PLEASE CONFIRM MY BOOKING

I might write it as:
12-gatsu 1-nichi no yoyaku wo onegai shita [your name] desu.
Nen no tame, yoyaku no kakunin wo onegaishitai no desu ga,
orikaeshi o-henji kudasai.
This is [your name] who asked for booking on Dec. 1st.
Just to be sure, I would like to ask for booking confirmation, so
please reply (with confirmation).
