I will be traveling for 13 days in southern and central Hokkaido (Hakodate/Niseko/Asahikawa/Sapporo) self driving, may I know which pre-paid 3G/4G SIM plan for foreigner works best (coverage/price etc) for my purpose, and how/where can I get them? I have unlocked iPhone 6s+ that uses nano SIM.

Try Docomo. I can't recommend a specific rental company as my only experience has been renting portable WiFi. Through Docomo we had good coverage even on the ferry between Wakkanai and Rishiri Island.
