On Japanese TV pranks shows, where they mess with people jokingly, Is it all setup (fake) or is it real. Also has anyone they setup every got hurt?

They are fake. These programs are not live - but taped in advance with all participants reading and following their script directions - the exact dialog, expressions, movements, camera position, lighting, etc.

Depends on the show. Some are fake, some are real. Some people probably did get hurt. There's only a few TV networks in Japan, and they have all the power and resources, so they do pretty much whatever they want.
