If there is no direct train, then you need to transfer once. Q2 I recognize the train is going to be separated into two at Tokyo station. ex. 6cars goes to Ikebukuro direction and the rest to Yokohama. When a transfer is needed, a case may be where the earliest Yokosuka Line train you can take for Yokohama departs from a neighboring platform, I presume.
