Planning our tirp in Japan

Hi everyone
I am planning to Japan with one of my friend next year Early March for 21 days, i guess we will be using the 21 days JR Pass.
We decied to base in Kyoto for temple tour for 4 days, before we go for our route trip.
(temple,Gion, Arashiyama and Ohara)
We will check out of the hostel on 5th days.
But we not sure which to start first, our last day we want to end up in Osaka or Kyoto for Kansai Airport.
a. Hiroshima/Mijiyama Isand - 2days
b. Nara - 2 days
c. Kinosaki onsen - 1 day
d. Hakone - 3days
e. Tokyo Disney Land & Sea - 2 days
g. Tokyo - shinjuku/shibuya, etc. 1 days
f. Takayama - 2days
g. Shirakawago - 2 days
any advice would be happy to receive.

Where do you enter Japan? Kansai Airport?
If yes, then the following order would be convenient:
Osaka/Kyoto/Nara - Kinosaki - Hiroshima/Miyajima - Hakone - Tokyo - Takayama - Shirakawago - (via Kanazawa) - Osaka
