Places to cover :
1) From Haneda airpot to Sakura Ikebukuro

2) Move around Shibuya, Shinjuku, Asakusa, Ueno, Harajuku,Kama, Roppongi

3) To Karuizawa,Kamakura

4) From Sakura Ikebukuroto Five Lakes, Mt Fuji

5)From Five Lakes to Hakone

6)From Hakone to Tokyo Bay

In terms of train lines/areas, the pass covers most of the places you plan to cover, except for Asakusa, (where is "Kama"?), Roppongi, and within Hakone. I don't know what you mean by Tokyo Bay. But three days is not enough to visit all those places.
