Re-application for Shinsei Bank account

I applied for a Shinsei bank account via mail but I got a rejection letter today.

Thing is, I applied with the English application form and the rejection is... entirely in Japanese.

I kinda understood what I did wrong but there are some Kanji which stroke order I simply do not know and have failed to write correctly so I don't know what they want.

Do I only have to include my address, written in the same order as on my residence card? Or fill out anything else, like boxes to tick (info which I don't understand since it's in Japanese...), signature?

I'm sick right now and lost my voice, I would have called them otherwise. If I'm better on Monday I will go to the Roppongi Hills branch.

This letter says:
"- Please refer to the attachment for the identification document and (other) documents needed for the procedure to open an account.
- The identification and other documents submitted cannot be returned. Shinsei Bank will dispose of them after a certain period, with due consideration for privacy protection.

Please inquire with Shinsei Power Call with any question."

So at least this part of the letter does not tell even me what was missing :(

There are several Shinsei physical branches. Visit one of them with your documents sent and/or returned.

I don't think bank tellers speak English or any other language but I think fairy enough to understand what you want to say. Ask them gently and politely.

The address on application form does not match with your personal identification.

I think the address on your Jumin-hyo and bank application form is different. Make sure write your Jumin-hyo address on the form. Check it again.
