Marriage Report

I filled this paper out 3 different ways and my wife is really nervous about how its written.. could someone please give me an example or explanation how to fill out this out? We live 3-4 hours from Osaka from the embassy and we would hate to have to go back and forth more than once..

It's pretty straight forward. Where are you having problems? Btw, in case your not already aware, that's the translation form for the Kon-in Todoke Juri Shomeisho, not the Koseki Tohon, which can also be used to prove your marriage. The Koseki Tohon form can be found here:

In any case, just bring several copies with you. The embassy staff can help you fill it out if you make a mistake.

I filled out same document when I got married but the embassy provided me a sample to follow. Is your embassy didn't send you one? You can submit whatever you have in hand and the embassy staff will correct you. They will allow you to use () and write the correct one on top provided you sign beside it.
