First eye doctor visit 2017/2/23 12:19
I will be visiting an eye doctor clinic for the first time. I think I have an eye infection or my sight has gone down hill within the matter of a couple weeks.

Will they require me to fill out forms? I don't know any Japanese, but I can use Google Translate to see what the form asks. Is it ok to write in English or do I have to write in Japanese?

Usually a simple form with your name, address, etc. and also any pre-existing conditions. About filling it in English, ask.

They ask you to fill a form contain your full name, residential address, phone number and date of birth.
Do not forget to take your insurance card.
If you can not write Japanese, receptionist normally assists you to write.
But, for first time visiting a doctor, I would recommend you to go with a Japaneses speaker together, the second time visit, you can go alone.
