How to make Icoca commuter pass

I'm just in the beginning of studying language in Kyoto. From my apartement to my language school I need to take train everyday so I want to make a commuter pass to get a cheaper price. I already have an Icoca card but it doesn't have the commuter pass yet. How can I get a commuter pass when I already have an Icoca card? I read somewhere that I have to go to midori no madoguchi office, filled the commuter pass form, and give it to the staff. I already went to Kyoto station and went to the ticket office but I don't understand how to fill the form since I can't read kanji yet. Please help me.

Also ask your school if you are entitled to a student commuter pass. It depends on the gkindh of student you are.
