Early Sept Hokkaido 13 day Self-Drive 2018/7/5 07:58
My gf and I have visited Japan before visiting Tokyo, Kyoto, Osaka, Takayama, Kanazawa, Kinosaki Onsen, Himeji, Miyakojima and Ishigakijima. For this trip we want to enjoy Hokkaido in the first few weeks of September. We love cities, beautiful scenery, hiking, cycling, meeting people and are really into great food. I speak basic Japanese.
Oh and we'll hire a car for the trip.

I've been trying to put together an itinerary and have come up with 2 different ones, I'd really appreciate suggestions on where we're going wrong, where to stay less time, more etc. or even huge changes.
All help will be gratefully received.

We fly from Switzerland to Beijing where we stay 4 nights before flying to Chitose, we fly out from Tokyo Haneda.

Itinerary 1 Shiretoko/Akan/Daisetsuzan/Sapporo/Rebun-Rishiri

Monday 03.09.18
16:35-17:20 Flight Sapporo to Memanbetsu
18:00-20:00 Drive to Otaru

Morning : Boat Cruise
Afternoon : Shiretoko 5 Lakes

Wednesday 05.09.18
Morning : Drive the Shiretoko Pass

Well, it depends on the one's preference, but I prefer Rebun than these two onsens.

Because you love hiking, I reckon the first itinerary would be more suitable. I haven't gone to Akanko Onsen so I can't vouch for it but visited everywhere else. You'd love Rebun and I think you'll be in your element if you have an extra night there.

Also I'm assuming you meant Utoro not Otaru.

Don't drink the Abashiri beer.

Yes, I meant Utoro not Otaru but couldn't edit the post.

So, you'd suggest skipping Daisetsuzan or at least shorten it to have more time in Rebun?
Is it better to stay in Rebun or Rishiri?

To be clear, we're not hardcore hikers, prefer 2-4 hours rather than full day hikes and really want to enjoy the nature, relaxation, cycling etc.
Probably the most important thing to us is great food and meeting people, not an issue in Japan!

I would stay Rebun as long as possible. Honestly there is a risk that every day is rainy, but if you stay long, you will have more chance to have nice weather.

Day: Drive to Sapporo via Furano/Biei Hiking from Fukiage Onsen to Shirogane Onsen.

There is an Onsen in Shikotsuko. I would spend all in an order of Shikotsuko, Noboribetsu, and Toyako. You may consider Onuma Park, near Hakodate.

Overall, both itineraries are a kind of a must. You just have to pick one. You are from an inland country. So, I may suggest island hopping. If you are keen on seafood, Hakodate looks attractive. But just drop by Otaru, or other port instead.

Among Japanese, driving along the coastline is rather popular. I think of a route of Otaru, Wakkanai, Abashiri, Bihoro Pass, Rausu, Utoro, and Akan, making a cross at Mashu. Cape Kamui in Shakotan Peninsula, Kaminoko Pond in Ura-Mashu, and Lake Onneto are popular destinations in September.

There's a variety of hikes you can choose from on Rebun and Rishiri, with various levels of intensity. We did a half day hike with a baby and preschooler in carriers so I'm sure you can manage ;)

We stayed at an affordable minshuku on the southern coast http://www.hamanasu-rebun.com/en/en-index.html
They picked us up from the ferry port and met us there with our bags (which they held for us during the day) for our departing ferry too.
