I was thinking, comparing my home country, in Japan practically all public toilets are free of charge. I haven't seen if every toilet come with paper though but is there never any problem with vandalization? Who pays for the maintenance of the toilets over time? I mean, I guess running them smoothly and providing water is not free.
I am always amazed about the convenience of having toilets around. Finding toilets so easily, even on many short distance trains, here is nearly impossible. Or they come with a fee.

I have never seen a paid toilet in Japan. I am guessing that we are all paying for them in the form of taxes or train fares etc.
I can only speak for men's side: Most (90%+) are clean. All are supplied with toilet paper. All have faucet/sink for washing hands, but less than 25% have soap. Most don't have paper towel to dry hands, but about half are equipped with hand dryer.
I have heard women's side is better equipped.

Kamikochi in Japan is nearly all paid toilets. You are expected to pay on the honor system, but nearly every toilet in Kamikochi requests a fee.
