I want to go to Hong Kong and China for a weekend.
Do I need a visa or any special paperwork?

You will need a visa for China for sure. Hong Kong possibly not - I know I've been to Hong Kong with my Canadian passport without the need for a visa, but that was many years ago - the rules may have changed since then.

Do you know where I can get information how how to get the visa?
When I googled, it said I had to get a visa from my home state...
but I'm living in Japan.

If you are in Japan and need to get a visa from your home state, that suggests checking/consulting with the US Embassy.
