Most trains will be running all throughout the night from New Year's Eve to New Year's Day. The Keio line will run at 15-30 min intervals, and the Yamanote line every 12 min.

Since the OP is asking about Disney back to Akihabara, I assume the line that is referred to is "Keiyo" line, not "Keio."

Re: from TDS, on N. Y. Day 2019 before dawn (1) 2018/12/12 12:31
Have you secured New Year's Eve Passport tickets?
On New Year's Day 2019,
a normal Tokyo Disney Park ticket
will not let you enter the Park until 2 am.

Extra trains will be available before dawn on some lines,
such as JR Keiyo and Yamanote Lines, subway Hibiya Line
and Disney Resort Line.
IC transportation cards of the ten major brands
(e.g. Suica, PASMO) can be used on these four lines.
Depending on timings,
your route from TDS until Tokyo Station may be crowded.

JR Keiyo Line: Section Kaihimmakuhari -- Maihama --Tokyo
: at intervals of about 15 to 30 minutes, from 1:00 until around 5:00.
JR Yamanote Line (Inner and Outer Loops)
: at intervals of about 12 minutes, from 1:00 until around 5:00.
Tokyo Metro Hibiya Line
: at intervals of about 30 minutes.

The JR transfer in Tokyo Station
from the Keiyo Line to the Yamanote Line
is practically a long way to go both vertically and horizontally.
(You go through Yaesu Pathway,
which is equipped with moving walkways, escalators and elevators.)

Akihabara Station of JR is tricky.
Disembarking from a Yamanote Line train,
logically you can go to Showa Dori Gate,
but you may get lost, unless you know in advance a good route.
It sounds better to head to Cental Gate, which is found more easily.

Alternatively,
you can make a short transfer in Hatchobori Station(s)
from JR Keiyo Line to Tokyo Metro Hibiya Line.
(There are two staircases on the way.)
Subway Akihabara Station is a little closer to your hotel
than JR Akihabara Station.
However,
New Year extra trains on the Hibiya Line
will depart at intervals of only about 30 minutes.

Will there be a little kid among you travelers?
Then, take care so that he/she would not catch flu or bad cold.
(Convenience stores sell disposable pocket warmers [ kairo ],
though they may be too hot for a little child to handle.)
Also, I recommend you will prepare some raingears.
