Where can I claim it ? City tax?

I suppose you can start with the person who issued it which would be your then client/employer.

源泉徴収票 is a private document (not having the same power of the official one).
since what you are asking is too old, your (previous) company may refuse to do (highly possible).
you can have the official document, 所得証明書 from city office where you lived in 2014.
