How to claim tax payment after lump sum?

Hi
Need help!!
I've just received the "Notice of Entitlement": for the lump sum withdrawal payment and "Notice of Remittance": for the lump sum withdrawal payment. I've read from many websites about to claim the 20% that is deducted from the payment. They only state to submit the original copy of the notice of payment and the document that I nominated someone to be responsible for this. My question is which original copy to submit because there are? I will have to send from my hometown to my friend in Japan.
