in the Lean Manufacturing world - is it "Poke Yoke" for fool proofing or "Poka Yoke" - i've heard "Poka Yoke" means something else..please help!

I think it should be "poka yoke."
"Poka" is short for "poka misu" (simple, careless, or absent-minded mistake/error often made or inevitable with people) and "yoke" comes from the verb "yokeru," to avoid. So put together it will be "avoiding/proofing against mistakes."
