It means he his thinking over various things. Depending on the (your) exact situation, he could be thinking of ways to get rid of his wife, or maybe he could be thinking about whether to go and see a doctor about his haemorrhoids. Who can say?

I would tend to interpret it as meaning that the guy is a bit reluctant to give an answer because he has a lot of things to consider first.
