Can anyone help me out with this one, because I have a doctors appointment but, dont know how to express myself,in japanese,so here it goes:
I have an appointment at 4:00pm today, I take this medication for hypertension twice a day and I need a prescription to buy some more at the drugstores.

- Kyou yoji ni yoyaku shite imasu.
I take this medication for hypertension twice a day
- Kono kou-ketsuatsu no kusuri wo ini-nichi ni-kai nonde imasu,
and I need a prescription to buy some more (of the same medicine) at the drugstores.
- Soshite, yakkyoku de onaji kusuri wo kaitai no de, shohousen ga hitsuyou desu.
