For hair removal I also recommend, on a good personal experience, the Azabu Skin clinic in Ebisu. Just google the name.

Hi I recommend Epi Salon. It's a very well known chain and it was recommended to me by several of my Japanese friends. It's slightly more expensive than some of the other less known places but it's got solida reputation. It has several branches in and around Tokyo. http://episalon.com/index.html
