Polish restaurant in Tokyo?

I've heard rumors of the existence of a Polish restaurant in the Kichijoji area.
Being from Chicago originally, I could really go for a fix sometime. Can anyone verify that this place is there and provide me with more info? Name / location / etc? Google search has come up mostly dry so far.

As far as I've checked, there's only one Polish restaurant in Japan and it is located in Nagoya.
In Tokyo, there's one restaurant which serves Czech cuisine, and three for Hungarian food.
They are different, of course, from Polish food but you might find something similar in those restaurants...

There is a Polish restaurant in the middle of Shibuya, Chopin Night. It is listed as a bar but they serve a lot of Polish food.
