First trip to Tokyo

Will be visiting Tokyo for the first time in mid December. Will be going there with my wife and my 17 year old daughter. Please suggest an itinerary for me and also provide some transportation tips to get around.Will be staying for 6 nights. Please also suggest other excursions outside of Tokyo. Thanks.
