Day 10 - Travel from Kyoto to Hiroshima. Spend rest of day/night in Miyajima. Stay in Hiroshima
Day 11 - Spend day in Hiroshima then travel from Hiroshima to Tokyo. Stay in Tokyo

The trip to Nikko is good, and hopefully the weather will be good for viewing the waterfall.

Heading south, I would recommend staying in Kyoto. it's easy to day trip to Osaka if you want, and there are many more things to see/do there, including being easier to get to Nara and the trip to Hiroshima (without changing trains at Shin-Osaka). Also, it would mean one less hotel change.

For day-12 (Fuji), if you're on the right side of the train heading south (day 6), you will probably get the best view of the mountain as you head past. You could probably better use the day back in Tokyo. Alternatively, if you are on a rail pass - jump on a train for a daytrip to Nagano or something like that.

Enjoy the trip!
