What is there inside the Kasuga Taisha inner sanctum for which they charge 500 yen? Is it something completely different from what you see in the outer area?

Yes. It is like looking at a hotel and entering the hotel. Outside you see the architecture.
E.g. Structure, windows
Inside you see the furnishings and the interior architecture.
E.g. Lobby, pool, restaurant, rooms, pillars, wall paper, paintings
