Contacting La Qua

I'd really appreciate some help using this Japanese website....
I really need to send an email to La Qua spa/onsen in Tokyo. However I can't see anywhere on the website with an email address. If anyone can look at the Japanese site for me and find an email link or email address it would be very helpful, so I can get the address to email them.

On the website they provide a phone number and online forms depending on the type of inquiry:

I'm afraid to say that Japanese site is not user friendly at all, too.
I can't find any contact address like inquiry form or email address, except for Uji mentioned.
In a word, they said "make a phone call".

As I suspected. I didn't know about the inquiry form - that might work.
