"O-namae wa?" would be the shortest way to ask "Your name (please)?"
"namae" means "name" "o-" is for showing respect, so "o-namae" points to the name of the person you are talking to "wa" is a topic marker, so it is like "As to your name?"
Full sentence (rather literal translation) would be: "O-namae wa nan desu ka" with the "what is...?" part as well, but the sentence conveys the full meaning without that part.
