Not that I can recall. They need to lean out to confirm safety, and I'm sure they're professional enough to know how to do it.
On the other hand, there indeed have been "accidents" where conductors had gone to the restroom and missed their trains. Things happen.

Wow, for real? How did they solve that? Did they jump on the next shinkansen trying to pursuit the lost train? :)
