Me and my fiance are both Filipinos. We went to Japan last February and found out that my fiance is pregnant. She now have biweekly checkup accompanied by her mother, who is a resident, and her stepfather, who is a Japanese. my fiance only has visitor visa. Is it possible for her to have a long term visa? what are the process that we need to take?

it is possible but you need to talk with the embassy about the process.
Since you are asking to apply a visa it means you do not have any job or other self support and that you will have to use savings or be depended on the parents.

This include also no health insurance, how will this be payed?

Immigration can see as risk for stay.
Please keep in mind that having a baby delivered in Japan does not mean she or he will be Japanese. Since both of you have no Japanese nationality.
