Does JR East Pass cover Tokyo/Shinjuku to Nagano and Toyama to Tokyo/Shinjuku?
If not what portion and what is the additional fare?

Tokyo-Nagano is fully covered.
Tokyo-Toyama is not covered between Joetsu-Myoko and Toyama.

The additional one way fare for the section between Joetsu-Myoko and Toyama is 4530 yen for a non-reserved seat on the shinkansen.
