when your payment to hospital was a small amount, like 40000 yen, you already signed the document in which you agreed that the hospital took 420000 yen. your actual payment should be 42+4=46, 460000 yen. they took 420000 yen from health insurance, instead of you. finally, you paid the resting amount, 40000 yen to the hospital. many people use this system. when you paid all amount without using this system (but, it is highly unlikely), you can get a full amount from health insurance.
