Incident Report after car crash?

I had a serious high impact crash in a rental car last week. I have fractured 5 ribs, collarbone, sternum, 4 spinal/lumbar bones, chest Haematoma, severely bruised knee caps, serious lacerations on left side of body. Ive been laying completely flat/still in the hospital bed since last Saturday. I need an Incident Report for Travel Insurance and the car insurance. How do I get that please? (And no it is NOT through the attending or district police station) Got the Australian Embassy involved now but they too can’t get a release either! They are also frustrated. My case/claim with Travel Insurance and Car Insurance can’t proceed until I have this report.

I have no experience. but, I think official accident reports are issued only by the official place, like police.

Was the accident reported/recorded by police?
then the next step is to get the Certificate of Traffic Accident 交通事故証明書 .. to proof that the accident has occurred. That report is not issue by the police but by (

you/agent/authorize person need to make application for the said cert., usually insurance agent will handle this. Application form should be available at the police station.

Well I now have the Accident Certificate as described above - it was absolutely fine with the Japanese based insurance companies involved but my claim with my Australian based Travel Insurance company ‘has been rejected due lack of detailed information as per their terms and conditions of our policy’. I asked them what else can I do - you have Japan based agents that can help me right? They would not give me any answer.

To clarify there were 2 police cars in attendance of my accident, 2 fire trucks to extract me from the car, 1 ambulance.
