How do I address a letter to Japan?

Hello~ My friend recently went back to Japan, and I want to send him a letter. he gave me his address but I have no idea how to send it, this is how he gave it to me:
takamasa 11-11-11-111 fukasawa setagayaku tokyo 111-1111 japan
All the numbers are "1"'s for privacy. Thank you very much!
