Opinion on Itinerary for 3 week visit

Hello,
A friend and I are planning a 3 week trip to Japan and i would like an opinion on our rough itinerary.
We are planning on visiting the first three weeks of April.
2 nights in Hiroshima, 4 nights in Kyoto, 4 nights in Nikko and 9 nights in Tokyo with several day trips from Tokyo.
We are planning going on several day tours but mostly are planning on discovering alot on our own.
Im wondering if you could offer some suggestions on places to go see, things that need to be done or places to avoid.
Also wodnering if it would be worth getting a 3 week rail pass or if it would be cheaper to pay for train rides individually.
