To Tokyo, 5nights, Hakone,2nights Osaka 3nights. Wish to visit Disneyland, Mount Fuji, Hakone and Universal Studio. What is best mode of transportation to these places.

I assume that with "Mount Fuji" you refer to the Fuji Five Lake region.
I recommend the following modes of transport:
Tokyo - (bus) - Fuji Five Lakes - (bus) - Hakone - (train) - Kyoto - (train) - Osaka
