In Nov I plan to visit Japan. Is it possible to travel from Hakone to Nagano without getting back to Tokyo?

Yes, however it will take much much longer.
If you are going via shinkansen, going through Tokyo takes roughly 2.5 hours.
If you go from Hakone (Odawara) through Nagoya then a limited Express train up to Nagano the trip is 5.5 - 6hours

Geographically speaking, the most direct route would be as follows:
Hakone - (bus; 15-60 minutes depending where in Hakone you start from) - Gotemba - (bus; 1 hour) - Kawaguchiko Station - (bus; 1 hour) - Kofu Station - (limited express train; 75 minutes) - Matsumoto - (train; 1 hour) - Nagano
For a total travel time of about 6 hours.
