What would be the fastest route on train from Mt. Koya-san to return to Narita airport for 5:30 p.m. flight to US? Too rush?

You should take the IBEX flight, that leaves Osaka's Itami Airport for Narita Airport at 13:30, arriving in Narita at 14:45.
It takes 3.5 to 4 hours to get from Koyasan to Itami Airport, so you should leave Koyasan around 8am.

Much thanks for the advice. I plan to purchase 14 day JR pass and wanted to use pass instead of flying back to Toyko to avoid spending additional $. :)
I revise my 14 day (11/20 - 12/3)itinerary, I love your advice on it 11/20 Tokyo 11/24 Takayama 11/26 Kyoto 11/30 Nara + Mt. Koyasan (convenient to go from Nara or better from Osaka?) 12/1 Osaka to Toyko 12/2 Toyko (maybe daytrip to Nikko) 12/3 depart

If leave Koyasan around 7 to 7:30, you sould also be able to catch your flight if you take trains via Osaka and Tokyo.
Rearranging your itinerary is probably the better solution.
11/30 Nara + Mt. Koyasan (convenient to go from Nara or better from Osaka?)
The fastest way from Nara to Koyasan is actually via Osaka, but if you have a Japan Rail Pass, you can save a few yen by taking JR trains from Nara to Hashimoto and change to Nankai trains there.
