denied or approved?

i did all the paperwork to get my wifes visa done. she is filipina. she got the visa back yesterday, w/ a piece of paper attached to the back, there was nothing saying that it was denied. but there is no new visa. she has one already in there from her 1st visit here. there is a reference # on the paper. is that a denial? would there at least be DENIED on the paper or something? or is it ok to buy her plane ticket? plz anyone, help me

tourist visa, when she gets here. im taking her on base, she'll get her dependant i.d. then we finish the command sponsorship package. and she will become s.o.f.a. status herself. but...cant do anything till she gets here. why would my wifes visa get denied. i just dont understand.
