Could someone please confirm - I can't get a definitive answer reading all the info on the site. In Tokyo, can I use an ICOCA IC card on all trains (subways etc), or just the JR lines?

You should have no problems at all when using it on JR trains in the Tokyo region. In fact many signs display both the Suica logo and the Icoca logo now (well at least at major stations). However on other systems they don't seem to have worked out the kinks yet to have it working.
My advice, invest in a Suica/Pasmo card anyway (you get the 500yen redeposit if you don't want to keep it as a souvenir)

I got these in the A-Z section at the top of this site. under transportation then under transportation in Osaka and transportationin Tokyo. this A-Z section is very very informative.
