you're a legal adult when you're 18 and can drive at 18, however, you can't smoke or drink until 20. So, I'm guessing 18 and up is ok.

Actually the age of majority in Japan is 20. You can vote, drink, and smoke at 20, but you can drive a car at 18. So the answer to the OP's question is most likely 20 years old. Of course, you can have plastic surgery done at a much younger age if you have parental consent and can find a willing surgeon.
