I am an American who intends to move to Japan. I have a Japanese girl friend, whom I been dating for 5 years. I only have an associates degree QUESTION: Where can I download the Spouse Visa form and how much is the fee?

Not with your current circumstances, no. But if/when you are married to a Japanese national, then you can apply for a spouse visa, which lets you live and work in Japan without restriction.

As for where to download a copy of the application form, if it's not available on the Japanese Ministry of Foreign Affairs site, you should contact your nearest Japanese embassy in the US or nearest Immigration Bureau office in Japan. I think the visa application costs something like around 4,000 yen.

You can't get a spousal visa unless you marry your GF.
