Hello:

What is the procedure for foreign visitors to get sales tax refund?

What I already know: 5% on purchases (excl. food/drinks) over Y10,000 in 1 store/day.

As I understand it from searching online, some department stores (Takashimaya?) will just take it off at the time of purchase. But what do you have to bring to show them (passport, itinerary, etc)?

What about smaller boutiques? For instance, if I were to buy a pair of shoes at Comme des Garcons, how do I go about getting a tax refund?

Is there a form I should a request from vendors to take to customs at the airport?

A passport should be fine. It has all the information that proves you are a visitor.

Sales tax refunds are only available for select licensed stores, typically large chain stores that deal with a lot of tourists. Small boutiques, unless licensed, cannot issue a refund.

No, unlike other countries, the airport is not involved since the discount is handled at each individual store. You will receive the 5% discount taken off at the time of purchase.
